Bellevue Crossroads Vet New Client Forms
Pets being dropped off for procedures requiring sedation or anesthesia should arrive at the hospital between 7:40 and 8:00 a.m. To ensure your pet’s safety and to facilitate better diagnostic imaging, food should be withheld after 9:00P.M. the night before your pet’s procedure. Pick up the water bowl/bottle before you go to bed. Please take your pet out for bowel elimination before coming into the clinic.
Please allow yourself 15-20 minutes as you will be required to fill out forms and speak with a technician before leaving your pet. If you have any questions or concerns regarding your pet’s procedure please call us at (425)746-PETS (7387).
